9/25/2015 Day 3 is HERE!

Hallelujah!!! Today is DAY 3 people!!! The day shit starts happening!!! I can not be anymore giddy!!! Lets DO this!

Walked in the office excited and not sure what to exactly expect. I could barely contain my smiles while sitting in the waiting room. “baby, baby, baby, baby, baby, baby, baby, baby

First we started with a blood draw, this will check my base line estrogen levels. Your level of estrogen can be a very good marker to tell you that your uterine lining is thin (you want that in the beginning), it can tell you how your follicles are coming along/growing, showing an increase in estrogen tells you your lining is getting thicker, your body is preparing for a baby, etc.

After the quick prick from blood work, it was onto the ultrasound (US). This isn’t the fun “above the tummy” ultrasound where you imagine seeing your baby’s profile type ultrasound. Everything is so small that every time they do a transvaginal ultrasound….you get used to these lol.

Today my ultrasound will be checking to see how many follicles I have on each side (in each ovary). Each follicle has the “ingredients” to produce a potential egg. This number is very important during ivf because the higher the number the better your odds for more eggs to make it to the end….though too many follicles means too many eggs and you may become overstimulated, which can be very painful and dangerous. This is called OHSS, Ovarian hyperstimulation syndrome. If this happens it doesn’t mean your cycle is bad or canceled…you just have to deal with mild to serious symptoms, usually happening after your egg retrieval.

There is also a case of having a low number of follicles, this is also not the best scenerio. Having a low number of follicles can mean your success rates for eggs making it to embryo transfer are low or you may only have a few that make it.

I’m “young” per the Dr. so this shouldn’t be an issue…well they were wrong!

My total follicle count is 6 on left 6 on right: 12 total

Not “low” but not good.

Total number of antral follicles
Expected response to injectable stimulating drugs and chances for IVF success
Less than 4

Extremely low count, very poor (or no) response to stimulation is expected.
Cycle cancellation is likely.
Possibly should consider not attempting IVF at all.

Low count, we are concerned about a possible/probable poor response to the stimulation drugs.
Likely to need high doses of FSH product to stimulate ovaries adequately.
Higher than average rate of IVF cycle cancellation.
Lower than average pregnancy rates for those cases that make it to egg retrieval.
Somewhat reduced count
Higher than average rate of IVF cycle cancellation.
Slight to moderate reduction in chances for pregnancy success as a group.

Intermediate (and “normal”) count
Response to drug stimulation is sometimes low, but is usually good.
Pregnancy rates as a group are slightly reduced compared to the “best” group.

Normal (good) antral count, should have an excellent response to ovarian stimulation.
Likely to respond well to low doses of FSH drugs.
Very low risk for IVF cycle cancellation. Some risk for ovarian overstimulation if a Lupron trigger is not used for final egg maturation injection.
Excellent pregnancy success rates.
Over 35
Very high count, watch for polycystic ovary type of ovarian response.
Likely to have a high response to low doses of FSH product.
Higher risk for overstimulation and ovarian hyperstimulation syndrome if a Lupron trigger is not utilized.
Very good pregnancy rate overall.

Before leaving the office the nurse assures me this number will be just fine, I should have all those follies (follicles) respond well to stimulation because again “I’m Young” (I’m 30) and everything should be great! My estrogen looks great as well!
